Observation form

In this section of the protocol, the evaluator will write down how the student responded to sounds in the environment in the far right column. If you have a Sound Level Meter, you would also note the level of environmental noise and the level of the signal noise in the first and third columns respectively.

In writing your observations, be as specific and detailed as possible because this may help with IEP development and/or program planning. For example, "In an average noise environment, child turned head in response to door slamming at a distance of 6 feet" is a better observation than "turned head".
