Procedures for Completing the FHI
- Select two or three persons who are familiar with the student to complete the Interview Form. If at all possible, it would be best to use the parent/primary caregiver and the classroom teacher (because of their extensive daily experience with the child) and one other professional (for example, AI Teacher, Deaf Ed teacher, speech/language pathologist, speech therapist, VI teacher, Occupational Therapist, Physical Therapist, or paraprofessional).
